ts Social Worker – Adult Early Support Team Adults Social Worker – Adult Early Support Team Location: Manchester Council: Manchester City Council Contract: Temporary / Full-time Tripod Partners are currently recruiting for an experienced Adults Social Worker to join the Adult Early Support Team at Manchester City Council. This is an exciting opportunity to join a team focused on providing early intervention and support to adults, helping people maximise their independence and achieve positive outcomes. The Role You will: * Manage a caseload of adults requiring support * Complete Care Act assessments and reviews * Identify individuals' strengths, needs and desired outcomes * Develop person-centred support plans * Promote independence and prevent escalation of need * Work collaboratively with individuals, families and carers * Liaise with health and other partner agencies * Assess and manage risk * Undertake safeguarding responsibilities where required * Maintain accurate case records and documentation About You The ideal candidate will have: * A recognised Social Work qualification * Current Social Work England registration * Experience working within Adult Social Care * Strong assessment and care planning experience * A person-centred approach * Excellent communication and organisational skills * The ability to work effectively within a multi-disciplinary environment If you're an experienced Adults Social Worker looking for your next opportunity, apply today
